WOOFOOD 2012 GIFT CERTIFICATE SWEEPSTAKES
1. NO PURCHASE, PAYMENT OR OTHER CONSIDERATION OF ANY KIND NECESSARY TO ENTER OR WIN THIS SWEEPSTAKES.

3. HOW TO ENTER
To be eligible for a chance to win a product prize (i) “like” WooFood on Facebook at http://www.facebook.com/pages/WooFood/128582510554234?v=app_194975693850063&rest=1, (ii) send an email with first and last name, email address and confirmation of age (18 or older); or (iii) send a four (4) inch by six (6) inch post card with first and last name, email, and confirmation of age (18 or older) to: 30 Dominion Rd. Worcester, MA 01605 (the “Company Address”), to the attention of: 2012 Gift Certificate Sweepstakes. Entry forms must be submitted between June 3, 2012 and June 17, 2012 11:59 p.m. EST for Online Entries - mail in entries must be postmarked by June 17, 2012 (the "Sweepstakes Period") in order to be considered an eligible entry. See PRIZE DETAILS below for further information regarding procedures for sending in eligible entries. No mechanically reproduced or photocopied entries will be accepted and entries generated by script, macro, bot, or other automated means will be disqualified. Neither WooFood Corporation (“WooFood”), nor any of its employees, officers, representatives or agencies is responsible for lost, late, misdirected, damaged, illegible, incomplete, incorrect or postage due/entries/mail, whether caused by participant or by any of the equipment or programming associated with or utilized in this Sweepstakes, or by any technical, human, or other error that may occur in the processing of entries. 4. PRIZE DETAILS
Nine (9) gift certificates to WooFood certified restaurants will be awarded as prizes during the Sweepstakes Period: (a) five (5) five dollar ($5) gift certificates to WooBerry Frozen Yogurt, (b) two (2) twenty-five dollar ($25) gift certificates to Nancy Chang’s, (c) one (1) twenty-five dollar ($25) gift certificate to Museum Café at the Worcester Art Museum, and (d) one (1) twenty-five dollar ($25) gift certificate to Flying Rhino Café and Watering Hole. All Prize winners must satisfactorily follow the entrance procedures identified in HOW TO ENTER above and submit your online entry form or mail your mail-in entry form by 11:59 p.m. EST on the last day of the Sweepstakes Period. Only one entry per prize, per person permitted. Any entries submitted beyond the limits specified herein, whether via entry form, mail-in or online methods are void, in WooFood's sole discretion. No prize substitutions allowed by winner. Prizes are not transferable. All federal, state or other tax liabilities (including any resulting personal income tax) arising from this Sweepstakes will be the sole responsibility of each winner. WooFood, in its sole discretion, reserves the right to substitute a prize of equal value if Prize becomes unavailable.
5. ELIGIBILITY
The Sweepstakes is open to legal residents of the forty-eight (48) contiguous United States and the District of Columbia who are 18 years of age or older at time of entry. Employees and officers of WooFood and its advertising and promotion agencies, and their immediate families (defined as spouse, parents, siblings, and children), and those with whom such persons reside, are not eligible to enter or win. All Prize winners from any previous WooFood sponsorship contest are not eligible to enter or win. There can only be one Prize winner per household. WooFood, in its sole discretion, reserves the right to investigate any entries that appear to have been fraudulently or improperly made, including, but not limited to, any entries created by computer programs or by any non-members. All federal, state and local laws apply. Void where prohibited. WooFood and its advertising and promotion agencies, are not responsible for printing, distribution or production errors and may rescind, cancel or revoke the Sweepstakes or the drawings based upon any such error without liability and at its sole discretion.
6. DRAWING, PRIZE ODDS & AWARDING OF PRIZES
Nine (9) prizes will be awarded during the Sweepstakes period. All prizes will be awarded by July 1, 2012. Each prize will consist of a cash or product prize with a range of value, as described above in Section 4- PRIZE DETAILS. Each Prize winner must have fully complied with the entry requirements found in Section 4- PRIZE DETAILS. Odds of winning prizes depend on the number of Eligible Entries received. Winners will be selected in a random drawing from among all Eligible Entries received. The drawing and the awarding of Prizes will be conducted by WooFood, or one of WooFood's designees, whose decisions on all matters relating to this Sweepstakes are final. Potential Prize winners will be notified by email on or about July 1, 2012 following the prize drawing, based upon information provided in the entry form. WooFood, or its designee, will make up to three (3) separate attempts over a three (3) business day period to contact the potential winner by email. If WooFood, or its designee, is unable to successfully contact a potential winner by phone and/or if potential winner fails to respond to the email notification within three (3) business days of such contact, such potential winner will be disqualified and forfeit his or her right to a Prize, and alternate potential winner will be selected from the remaining Eligible Entries. If a potential winner is unable to furnish verifiable and valid proof of age within three (3) business days after being notified of winning, an alternate potential winner may be chosen from among all remaining Eligible Entries received at WooFood's sole discretion. Unclaimed Prizes may not be awarded. Winners agree that WooFood may use their names or likeness for advertising and publicity purposes without any additional compensation (except where prohibited). All winners will be required to execute and return an Affidavit of Eligibility, Liability Release and (where permitted by law) Publicity Release within five (5) business days of successful notification, and entry into this Sweepstakes constitutes the winners' agreement to sign such releases. Non-compliance with any of the foregoing may result in disqualification and an alternate winner may be selected from among all remaining Eligible Entries, in WooFood's sole discretion.
7. GENERAL RULES OF PARTICIPATION
The receipt by winner of any prize is conditioned upon compliance with any and all federal, state and local rules and regulations. ANY VIOLATION OF THESE OFFICIAL RULES BY A WINNER (AT WOOFOOD'S SOLE DISCRETION) WILL RESULT IN SUCH WINNER'S DISQUALIFICATION AS A WINNER OF THIS SWEEPSTAKES AND ALL PRIVILEGES AS WINNER WILL BE IMMEDIATELY TERMINATED. By participating in this Sweepstakes, entrants agree to be bound by these Official Rules and the decisions of WooFood, which are final in all matters relating to this Sweepstakes, and waive any right to claim ambiguity in the Sweepstakes or these Official Rules.
WooFood and its advertising and promotion agencies, and the officers, directors, shareholders, employees, and agents of each (collectively "Released Parties") disclaim any responsibility for mechanical, technical, electronic, communications, telephone, computer, hardware or software malfunctions or failures of any kind, including, but not limited to: failed, incomplete, garbled or delayed transmission of online entries, traffic congestion on telephone lines, the Internet or any web site or lost or unavailable network connections which may limit an online entrant's ability to participate in the Sweepstakes, and any injury or damage to entrant's or any other person's computer related to or resulting from participating in or downloading any information necessary to participate in the Sweepstakes. WooFood reserves the right to cancel, terminate, alter sweepstakes duration, modify or suspend the Sweepstakes without notice. CAUTION: ANY ATTEMPT BY AN ENTRANT OR ANY OTHER INDIVIDUAL TO DELIBERATELY DAMAGE WOOFOOD'S WEB SITE OR PROMOTIONAL PARTNER OPERATED WEB SITES OR TO UNDERMINE THE LEGITIMATE OPERATION OF THE SWEEPSTAKES IS A VIOLATION OF CRIMINAL AND CIVIL LAWS AND SHOULD SUCH AN ATTEMPT BE MADE, WOOFOOD RESERVES THE RIGHT TO SEEK DAMAGES FROM ANY SUCH INDIVIDUAL TO THE FULLEST EXTENT PERMITTED BY LAW, INCLUDING SEEKING INJUCTIVE RELIEF. THE MASSACHUSETTS COURTS (STATE AND FEDERAL) SHALL HAVE SOLE AND EXCLUSIVE JURISDICTION OF ANY CLAIM OR CONTROVERSY ARISING OUT OF OR RELATED TO THE SWEEPSTAKES, AND THE LAWS OF THE STATE OF MASSACHUSETTS SHALL GOVERN THE SWEEPSTAKES. The use of any non-WooFood trademarks, service marks, names, likeness and/or voice (collectively, "Property") in connection with this Sweepstakes, or the award of any prize, is not meant to imply the endorsement by WooFood of the respective owner(s) of such Property, or any affiliation with or of the respective owner(s) of such Property, with WooFood or this Sweepstakes.
8. DATA COLLECTION
WooFood collects personal information from submitted Sweepstakes entries, which may be used in a variety of ways, including customizing features and advertising that appear on the Site. WooFood reserves the right to market promotional offers for WooFood's network of sites or via third party partners. Entrants may opt out of receiving future mailings or other information from WooFood by sending an e-mail to info@woofood.org indicating their preferences.

The names of winners will be posted at http://www.woofood.org/index.html through September 31, 2012. Alternatively, you may send a self addressed and stamped envelope by September 31, 2012 to the Company Address.
